AFH Residential Services provides a safe, comfortable, family orientated environment suited for personal growth and success of the individual. Two levels of residential support are provided.

Supported Living - licensed settings with 24hr supervision provides assistance to individuals needing extra support to maintain an independent lifestyle.

Independent Living – varied levels of support offered to those living in their own apartment or home to assist them as they enjoy an independent lifestyle.

Supported Living

AFH staff are dedicated to promoting independence in our homes. Respecting the dignity and choices of our individuals is our first priority.

Supported living provides individuals the opportunities to learn the techniques needed to gain a living style that is as self reliant as possible.

Our staff teaches meal planning and preparation, budgeting and shopping for household and personal necessities, personal hygiene, health care, community orientation and recreational activity planning and participation.

AFH staffs are also trained to supervise medical appointments, handle emergency situations, provide safe transportation to life’s events and provide assistance with personal care as required.

Family and friends are always welcome to visit.

Independent Living

Home and Community Based Services (HCBS)

HCBS consists of several different services provided by AFH. These services include living, habilitation, attendant care, and respite service.
Independent living - For those individuals living in their own apartment or home AFH provides supports tailored to the specific needs of the individual from 24 hour assistance to just a few hours a day.

Habilitation – Is designed to maximize the knowledge and skills for an individual to become as self sufficient as possible.

  • Provide training / assistance to meet personal, physical and social needs.
Attendant Care - To assist an individual to maintain a safe and sanitary living condition and personal cleanliness.
  • Showering, bathing
  • Dressing
  • Toileting
  • Transferring to and from wheelchair
  • Cleaning of the home
  • Laundry
Respite - Short-term care and companionship to support or relieve primary caregivers.
  • In home or out of the home
  • Participate in activities
  • Spend time with the individual
